In Order to continue, the Arizona Supreme Court requires that you send via fax or email the following:

Items to send:

1. A copy of your Arizona traffic ticket or complaint. (Sample of Complaint)

2. A copy of your current Driver's License. (Sample of Driver's License)

What Happens Next?

Once the documents above are received and are legible, your eligibility will be checked with the state system within 24 hours.
Once your eligibility is confirmed, your account will be activated and you will get a notification via email when you can proceed.
If you have not heard back from us after 24 hours, please contact our office.
